Built to HSE's own permit anatomy
HSG250 sets out what a permit form must carry. This one carries all of it — including the three things most permit forms leave out: a second-person authoriser check before issue, a cross-reference field for other permits live in the same area, and a proper suspension record.
That cross-reference field is not decoration. HSE's own case study on Piper Alpha identifies the failure as work under one permit affecting work under another with no cross-referencing between them.
The legal hook is real, and we cite it on the form
DSEAR 2002, Schedule 1 paragraph 6, is the only place in UK health and safety law that requires "a system of permits to work" in those words — issued by a person with responsibility for that function, before work starts, in hazardous places or involving hazardous activities. That covers hot works. It is printed on the permit.
Fire watch periods follow the Joint Code of Practice — check the current edition and your insurer's requirements against the period you apply.
